Objective: To report the case of a patient with Primary Sjögren Syndrome with neurological manifestations of demyelinating characteristic. Method: This is a case report type study, in which the information was obtained through a review of the patient's medical chart, interview with the patient, photographic record of the imaging exams and, finally, literature review. Final considerations: The neurological manifestations of Sjögren's Syndrome are rare and present a wide clinical spectrum, including demyelinating manifestations that simulate multiple sclerosis, besides optical neuromielitis. Treatment is often based on extrapolation from other autoimmune diseases such as rheumatoid arthritis and systemic lupus erythematosus, and advocates the use of immunosuppressants.
