We report a case of severe hypertriglyceridemia in a 61-year-old woman, who was, during a cardiologic routine consultation, diagnosed with severe hypertriglyceridemia, which is resistant to the prescribed treatment. She is retired, sedentary, has no healthy living habits and lives alone in her home. She has forwarded to the nutrition clinic and we suggested a therapeutic plan for lifestyle changes. The combination of lifestyle therapies were: physical activity, diet change and insertion in social life. Dietary recommendations followed the proposals for Brazilian dyslipidemia guideline updates with the addition of social interaction. In this case, the dramatic serum triglyceride levels reduction has been demonstrated with the combination of lifestyle therapy and body image improvements and return to social interaction. Thus, we believe that these additional actions were crucial to motivate the accomplish of proposed dietary plan and avoid the risk of pancreatitis
